Review of the 'lanolin paradox': lanolin sensitizes a notable share of dermatology patients (especially on eczematous, ulcerated, or otherwise damaged skin) yet is well tolerated in cosmetics applied to intact skin; lanolin-sensitive patients often show false-negative patch tests to pure lanolin, and 30% wool-alcohol patch testing is an unreliable detector.

Single-center retrospective analysis of 24,449 patients patch-tested with 30% wool alcohols (1982-1996): mean annual lanolin (wool alcohols) sensitivity rate was 1.7%; allergy was more frequent in women and older patients and highest among those with lower-leg/stasis dermatitis (6.0%).
